When AI helps care coordination and when it does not

For leaders evaluating AI around handoffs, referrals, follow-up, and patient operations.

Care coordination is full of language, context, timing, and accountability. That makes it attractive for AI and easy to overclaim.

AI helps when it reduces ambiguity, prepares the next human action, or surfaces missing context before a handoff fails.

It does not help when it creates another inbox, hides uncertainty, or makes ownership less clear.
